MS Formatting-Email sub

The more time I spend on this forum, the more mistakes I realise that I've made in the past. Now that it really counts, subbing a full ms ( requested revisions) to an agent, I know that the final judgement is coming soon and I want to get everything perfect.

I have a few questions I hope you guys can answer for my e-mail sub:

Double or single spacing?

TNR or Courier New?

Italics like this or _italics like this_ ?

Do I include the page thingy at the top (Writer 1-Writer 2/BOOK TITLE #) in the e-sub?

Is there large spaces between chapters?

How the heck do you show paragraph breaks when sometimes you do breaks to show a separation of time like:

Suzie fell asleep quickly


School was going to be nightmarish today--flocks of teenage girls would be begging boys to be their prom date, whether they were going out or not, there was the chem test she never did study for, oh and there was Jason. Jason always made her day worse.

Yes, the agent requested it. It's a full revision on a requested full. My co-writer was in charge of sending material the agent until he (the co-writer) forwarded it to me and I realised how incredibly unprofessional it looked, and I was just like, uhhh no. Let me handle this. Kthanxbai.
