For Those That Have Seen The Film - 'Vera Drake'

Just out of curiosity, when would you say the film's twist occurs?

I watched it on the weekend and then was discussing the film's plot with some friends and now I am curious...

Or would you say the film doesn't have one? That it intensifies but that there is no "twist" as such?
