Italics problem when converting to epub in Calibre

I'm trying to convert an rtf to epub in Calibre (9.3). However, not all of the italics show up. Some of them do, but not all of them. There seem to be more problems in the latter portion of the document, but it's rather random. This is not for publishing; I just want to proofread my novel on my ereader.

I have:
An rtf file I saved from my doc file - this causes the italics problem.

I can solve the italics problem by:

1. Copying everything from the rtf file and pasting to a new document using "keep source formatting", saving as rtf. Then converting this file. This rtf file is smaller, I think because it doesn't have all the heading styles information saved. The problem with the epub output here is that there is no table of contents. I have played around with a bunch of things, trying to get it to pick up chapter headings in Calibre, with no success.

OR

2. Saving to pdf in Word, then converting the pdf to epub, but this seems to cause all sorts of paragraphing problems.


So I need to:
1. Find some way to get all the italics to convert properly from the original rtf file - this is preferable.

OR

2. Find some way to get a table of contents from the second rtf file. If I have to select each chapter heading manually, that's OK, though not ideal. However, I cannot figure out how to do that.

OR

3. Prevent all the unwanted line breaks when converting from pdf to epub.


Any ideas?

EDIT: I have a file that appears OK now. I went through rtf #2 in Word, and designated all the chapter names as headings again, so I now have both a proper table of contents and all italics when I convert to epub.
But if anyone has any ideas as why I had problems with italics to start with, I would appreciate any input. I am worried that there is something wrong with my original doc file, and the rtf I saved directly from it?

I expect it is just an inconsistency in formatting -- how it was originally applied to different parts, and/or interpreted by Word when pasted or something -- that doesn't show to the naked eye. Word does some very sneaky hidden code, and it likes to try to outthink us but it doesn't read minds half as well as a well-trained pet mouse.

You can open an RTF file in a text editor and see all its guts ;)
