do I add a second book cover to my signature ? When I try to save a second different cover image, I get the error message that I can't add the same picture twice.

The problem could be that you're using the SIGPIC (uploaded picture) option to display your cover pic. And you can only have 1 of those.

Easiest way might be to upload your cover pics to Photobucket or similar image repository, then copy/paste the URLs into your sigline (with the IMG and /IMG tags wrapped around the URLs).

Edit: or maybe you can copy/paste the existing URLs from the multilingualmatters website if the images already reside there?
