What would you do if an agent listed submission guidelines as "Send query and SASE via snail mail?"

Do I just send the query letter and synopsis?

You just send the query and the self addressed stamped envelope -- no synopsis or pages.

Give the agent exactly what s/he asks for. Just a query--no synopsis.

I'd send a query letter and 5 pages unless the agent says something like "no pages --- only a query letter."

Normally you would just send the one-page query letter and SASE, as already mentioned. However, there's been some discussion in this forum about sending sample pages along even when they are not requested as part of a cold submission. The title of the forum is "Include a Few Pages with Query". It's worth checking out.
