It can vary - depends on what the contract said. In my experience, I have been paid a flat one time rate for use of the story in most instances and in one instance, I also get very small royalties.

Ditto, plus a copy of the anthology. If it was a prestigious enough anthology or publisher (I dream) then I would do it for just the flat rate and an awesome credit to list on my next query letter.
For print anthos, flat fee for one-time rights, and 1-5 copies of the book. For e-pubs, flat fee, sometimes with royalties and trib copy. Again, one-time rights only.
Not a way to get rich. You've got to think of short story credits as just that- credits and proof that an editor chose your work.
