Ok let me start over. An agent has requested the first 3 pages of my book.
On the first page, the title page, what do I include? What I have now is:
name
address
phone
email

title of book
word count
genre

Is that right - and is that the right format?
Thank you!

Personally, I wouldn't include a title page... or if I did, I wouldn't count it as the first page.

From everything I've read on the subject, when an agent asks for pages, they want pages of the actual story. So send the agent the first three pages of chapter one.

There's an example halfway down the page of this website illustrating how to format your first page:

Edited to add: I see from the other thread you started that you've received a request for the first three chapters, rather than first 3 pages... I wondered about that when I read your post. Anyways... thought I'd mention this, otherwise you'll be thinking my advice doesn't make a bit of sense.

Whenever I've sent chapters, I've put all the info you listed on the first page, then started Chapter One about 1/3 of the way down. That way, the info was there and handy, but not on another page, which seemed a bit much to me.

That's how I've done it, based on advice from somewhere. Probably one of the Writer's Digest guides (i've purchased so many over the years....)

I've always done a separate title page, listing everything you said. I don't count it as page one though, just an at-a-glance page of info. I don't think this is crucial though - as long as your details are on it somewhere at the front, it won't matter exactly how they're set out because so many people do it different ways. What they're looking for is the quality of writing. Good luck!
