OK, I'm getting myself confused. Would captain be capitalized in that sentence or not? I was thinking yes, because there's the implied last name.

"Permission to speak, Captain [Smith]."

"What is it, Lieutenant [Jones]?"

It's used as a proper name, so yes, it would be capitalised.

That's what we call my dad, and we always capitalize it. He's a boat captain.

As scarletpeaches said, proper name = capitalization.
