Proper Manuscript Format

Hi all. I have to supply copies of all my manuscripts for my agent today. What is the proper format for children's manuscripts? Do I include the word count and name at the left hand corner of each page?

Any help will be awesome! Thanks!

Format them like any novel. Just see the sticky here or in the novels writing forum.
