what is the required info for a title page

What is the prefered format and required info for a single title page included with a synopsis and first three chapters, please.

Go here and look at "Cover Sheet." They're the same thing. Good luck!

No wonder I couldn't find anything online when I searched!! Never thought to try under 'cover page'.
