Format for partials (as opposed to complete manuscript)

I've got a request for a partial and I'm wondering how its format should differ from the format of the complete manuscript.
First of all, am I right to assume that if your novel has a prologue, proem, or other such introductory text, you should omit it from the partial?
And that if your novel is divided into parts, you should omit any "PART 1" page?
Now, what I don't know how to handle is the manuscript's title page. When you're sending the first three chapters or 50 pages or whatever, do you include the title page (with your address, word count of the entire manuscript, epigraph, etc), or do you even omit that and just begin with the first page of chapter one, putting the title up on the top of that page?
I'm also wondering how to handle the dedication page. If you have one, do you include it when submitting the entire manuscript, or is it something that's added after the book is sold?

My opinion: No difference.

Imagine taking your full ms., pick the first 50 page (plus title page) and send that in the mail.

That's it.


Skip the dedication page altogether (even in your ms.), though. Your ms. shouldn't have that. It's a done much later...

When I was querying I just pulled the first 25 pages-
My agent asked for my full just from my query though - (as I added the first 5 pages (without title page) to my query and she had that to look at).
Most of my full requests by passed the partial.
