Hypothetical Question.
Let's say you have a character (Bill) who transforms into another (Joe) either by makeup or different actor.
When Bill becomes Joe, how do you handle CHARACTER NAME?
BILL
(as Joe)
Hi.
or...
BILL/JOE
Hi.
or...
Bill transforms into Joe.
JOE
Hi.
or something else....

i would identify it in the prose that joe is now bob.

I'd use "Bill transforms into Joe." which scores on ease of understanding.
You got some kind of Twin Peaks thing going on? Is this a cry for help?

Once you establish in your narrative that Jeckyl has turned into Hyde, you don't have to keep referring to that anymore. Just say:
JECKYL
blah blah blah
...
HYDE
blah blah blah
