release form

Hi all,

Wondering if any of you have used a release form when writing an article/book. My book includes many interviews and I want to have the particpants OK my use of their name, business and comments. Do I send them the portion of the book they are mentioned in? I want to be clear it's not for them to edit, but to verify information.

Any ideas?

What, for fiction? Or is this a non-fiction book we're talking about here?

the book is nonfiction. it's magazine style, with interviews. i need to get a written OK from the contributors.

also debating sending them the portion of the book i wrote using their questionnaire, or just the questionnaire.
