Decision-makers at Publishing Houses

How are the major publishing houses structured in terms of decision-making on manuscripts? In other words, once an agent pitches and submits the MS to an editor at a major house, how many people usually read it before a final decision is made? Is it just the editor him/herself, or is the decision usually made by committee?? And what's the general turnaround time for the editor to make an offer (or reject the MS)? Thanks!
