need advice on contract attached

Hi, I recently received this contract (attached) from a literary agency but haven't had enough experience with contracts to know if this is a valid and honest contract. I left off their names in order not to color any opinions.
Could use some sage advice as soon as possible.

I pulled the attachment because I goofed and left a name in where it shouldn't be. Now I don't know how to put the corrected attachment back it. Help! Cpt B Bear
