*The "Do Not Query Before" date posted by the publication has passed (ex. Do not query before 90 days have passed).
*You have reason to suspect either the submission or the response has gone missing.
Then I'd word it something like this:
Dear (Editor of Magazine),
I submitted my manuscript, entitled: X to your publication on the (day) of (Month) of this year. I am unsure if the submission made it to your publication. At your earliest convenience could you please inform me of the submission’s status? Thank you for your time,

If you are simply antsy about hearing back, and/or if you know they have the submission, and if there's no reason to believe they've already sent you a reply, don't query. Queries clutter editors' inboxes and take up valuable reading time. Only query when you absolutely have to.
