Question on Front Matter

Does anyone know about the protocol for writing certain parts of the front matter in an academic book?

1) Who determines which Subject Headings to use, the author or the publisher?
2) What do the abbreviations "p. cm." refer to?

I believe you'd put everything the way you envision it into your proposal, and the agent will give you direction if anything needs to be changed before submitting to publishers.

Go to the public library, and borrow any edition of The Chicago Manual of Style

Your individual publisher may very well have a preferred style wrt to headings etc. but you won't go wrong by following Chicago. It explains fairly clearly, and gives examples.
