Query letter newbie

I'm going to write my query letter tonight (considering my novel is about 85% done and will get done within the next month or less) and considering Ethan Ellenberg as my agent. Can someone help me with the format of my query letter? Where do I put my name? Where do I put my address? Do I wait to send them my MS until after they read my query?

don't send ANYONE your manuscript unless they ask for it specifically and follow all the guidelines to the letter...

a lot of agents get huge packages dumped on their doorstep and they rapidly turn them into shredder fodder because the agents only have time to read the query. If they like the query they'll probably ask for a few pages or chapters, depending on their needs.

always follow the guidelines and you can't go wrong!
