I have to disagree slightly with the 'going for gross-out'. IMHO going for the gross out because you can't scare your audience is a cheap, lazy way out. I'm not saying you shouldn't use it, but I'd rather pull it out in small quantities so when there is that full-blown nasty scene it is more effective. I prefer the psychological horror any day of the week. - but again, it's just my opinion.

Amen. There's nothing scarey about blood, gore and gross things. True fear is subtle and affects a person so that the feeling lasts longer than a momentary, "Ewww".
