Polite Wording for a Nudge

What's the most professional and politest way to nudge an agent or publisher or basically anyone who's reading your manuscript? I know opinions vary on how long to wait - I've heard 3-4 months. But I'm really wondering how to word a nudge to avoid offending someone.

"Dear [Name]",

I submitted my query/novel/etc. to you on [insert date here]. Can you let me know if it is still under consideration?

Sincerely,
[Your Name]"

That's what I do, at least. Just don't be rude and you're fine--this is a simple business letter looking for very specific information.
