at least one appendix

Is there a standard order for all these things?

Well, I made a command decision. If an editor/publisher wants to change it, he can.

Generally, the publisher takes care of the index, though none of mine have been finished in time for me to proof them. Worksheets might be in or at the end of the chapters (?) to which they relate. Appendices usually follow the main text, and are followed by the glossary and the bibliography. Where the bio goes depends on the publisher's house style, but is usually at the end, behind everything else.

You're right, though -- the publisher will let you know where in the book they're putting everything.
