It's an awkward structure, but "whom" is correct. If you broke it out into two sentences, it would be "Whom does this affect?" and "With whom does this occur?"

Remember, you use "who" when the answer to a question would be "he", and "whom" when the answer to a question would be "him".

Whom does this affect? Him. With whom does this occur? Him.

The correct usage is: Whom does this affect? With whom does this occur?

But in dialogue or conversational narrative: Who does this affect?

I was wondering about the who/whom thing also.

Thanks for pointing out that it's okay to use who in dialogue/conversations. This is good to know.

Correct or not, my writing or not, I think I'm going to reword and get rid of it. I have not seen anyone who likes it yet.

I tend to think people who say "Whom does this affect?" grammar snobs. ;)
If one were so inclined, one could peruse my User Profile, and find that I do, in fact, refer to myself as a grammar pedant. It is both my greatest failing, and my greatest asset.
