I heard about this on the radio tonight. On this day in 1942, 492 people were killed in the Cocoanut Grove fire in Boston. That is not a misprint. This tragedy was a lesson in how to make a bad situation worse -- from overcrowding to the use of flammable decorations to mass panic that resulted in revolving doors being jammed to exits that had been welded shut. On the positive side (such as it is), it did result in improved fire codes.
