Capitalizing royal titles

I know when you write "Queen Ann" the royal title is capitalized but what about the following sentence.

"Yes, my lady." Does "my lady" need to be capitalized?

I don' think so. I think lord and lady are only capitalized when used with a name - Lady Diana, Lord Such-and-Such.

But you may wish to put an "e" in "Anne"--at least if you're talking about the real Queen Anne.

Another question along the same lines. Should it be "the queen of England" or the "Queen of England"

The Queen of England. Queen is always capitalized when referring to a monarch (as opposed to a beauty queen).
