Say you sell a book to a small press. They deal mostly in ebooks, but offer trade paperbacks as well.

But the paperbacks are printed by a vanity press like Booksurge.

What do you have here? The book's gone through editing and all that, and you've paid nothing, just like a "real" publisher. Have you been published "for real", or is it just selective vanity publishing done by a middleman? Does it matter how a small press goes about printing?

IMHO the publisher is the small press, book surge is the printer. So you have been published (both e and POD) by a small press--no self/vanity etc about it.

For me, a publication credit results when (a) someone else pays all costs associated with publication and (b) you become entitled to a payment based on actual sales/expectation of sales.

For others, only an offset print run will do. The SFWA, for example, will not admit members based on an ebook or POD credit, to the best of my knowledge.
