Do you italicize movie or tv titles that come up in dialogue?

Methinks since you're using a Courier font, you would underline not italicize anything you think ought to be emphasized, so it's more easily discernible. Crap example just for fun's sake,

The usual thoughts: is it really necessary, use sparingly, etc.

You should italicize it whether it's in description or dialogue. It doesn't have anything to do with who's saying it--the character or the narrator. It's just a convention to show that it's a title. In the same way, you would italicize foreign words used by your character (as long as they aren't so common that they are listed in the English dictionary).

It is also my understanding that things should be underlined, not italicized.

Basically, for whatever reason, the industry seems to want its scripts to appear as if they'd been typed on an old manual typewriter.
