Heart attack causing drug with a twist....

I need a man (mid 30s, not very healthy, sedentary lifestyel, probably a heart attack risk anyway) to look as if he was killed by fear - a heart attack caused by shock from seeing something frightening.

So, what I need is a drug which:

- can be ingested in food
- does not have an immediate effect but significantly increases heart attack risk from other stimuli
- May need to be given to him in small doses over a number of days to have an effect.
- can be detected in a post mortem but only if they bother to look for it.

Any ideas? I'm stumped and may therefore have to make something up if it does not already exist... All answers will recieve gracious thanks!

Just an idea off the top of my head. Make him a smoker and then have him ingest more nicotine (easily obtained in something like Nicorette); it would make his heart race like a choo-choo train. I would have to do more research if I were using it.
