I'm guessing its either a Pontiac that thinks its a person or a transgender or transsexual man.
Its transsexual man who had the sex change from female to male. Oh and when i say insane, it means the drama. I don't think transsexual/transgender are insane.

I take it you haven't read much modernistic stuff from the first half of the 20'th century? I think we've taken "insane" about as far as it'll go while still being coherrent. Just an observation.
