Psychologist questions

I have a protagonist who is going to find out some strange things about her childhood from files of an evaluation a psychologist did on her when she was a child.

What would this evaluation be called? How would the report refer to the child? Patient or the child's name?

I can't answer what the evaluation would be called. Different hospital/clinics have different names for forms so as far as I know, you're clear on that regard. Names for forms are usually extremely utilitarian, though.

The psychologists who work in "my" clinic use the patient's first name in their reports, FWIW.

Google Susana Kaysen and see the crap she went through. I'd recommend Girl Interrupted to you too. She defines crazy like no one has. It's a great book. It has photocopies of all the forms too.

A lot of stuff would be on the initial intake form. The person could be referred to by name, or "clt" (for client).
