It varies for me, as well. But that mainly comes from how I come up with my characters. I'm a very visual person, so I tend to 'cast' my characters (the main ones, at least) first, then figure out their backgrounds.
How does this relate to the topic? Well, most often, I'll have an idea first, then I'll wander around in my head a bit until I find the right 'person' for the story. But occasionally, I'll come up with a character- background and all- and he/she will sit in my 'bio' file for an hour, a month, sometimes longer, until I find the right story to tell for him/her. I just have to wait for all the voices in my head to agree on what that is.
