When it comes to publishers filing for copyright with the US copyright office, as far as I know, a publisher will use the author's name when it comes to the section marked: "Copyright Claimant", and then put their name under: "Rights and Permissions".
But what happens if the publisher puts their name as Copyright Claimant too, yet when it comes to the contract, there's nothing in that contract that says the publisher will assign copyright in the publisher's name?
Have they moved beyond Rights and Permissions, beyond exclusives, and gone into claiming complete copyright without the author's knowledge (maybe by mistake)?
