I've never actually kept a tally chart on this subject, but I'd say that "nutsack" is the answer choice that I've heard most often.

Also, I'll go on record to say that "sac" (without the "k") seems too medical, too clinical, too cold.

Four out of five men currently in my house vote for sack. The fifth just shook his head at me disdainfully.

Bag is what I would go with.

Alternatives:

Sock (only use this if there is a mention of hazing.)
