I did a search and couldn't find it. My skills may not be that good though. Is there a example manuscript on AW that has the front page with the title page, etc.?

Read the guidelines to wherever you're submitting to and do what they ask, but this is is a good example in general. Notice there are examples for both novels and short stories, as well as other formats.
